Reserve Article/Chapter Request Form
To submit an article or chapter for DUMC Library Reserves, please complete and submit the form below via the Web. Completed forms may also be printed and attached to reserve requests containing personal photocopies of journal articles or book chapters, which must be submitted in person or via mail (Duke Medical Center Library, Article/Chapter Reserves, DUMC Box 3702, Attention: Suzanne Jessup). Forms submitted over the Web will be retrieved once daily, Tuesday through Saturday. Please see the Faculty Submission Guidelines  for processing times.
